Students will present the results of their practicum project in the form of an online gallery walk.
Provide an online gallery walk Power Point presentation which explains the purpose of your change project and addresses all of the below listed elements

- Describe the major points of the systematic review.
- Identification of practice issue and scope of problem.
- Identification of target audience/aggregate.
- Implementation change project which addresses the problem. Provide a complete description of the project and how it unfolded.
- Describe the evidence based interventions.
- Describe the interdisciplinary approach used for the project.
- Describe the ethical and cultural considerations of the project.
- Describe how information technology was used or will be used in the project.
- Identify the measurable outcome(s) that will be used to determine effectiveness of the interventions.
- Describe the communication plan for the project.
- Explore how this project improved the quality and safety for the aggregate.
- Describe recommendations for future practice.
- Summary (Describe the major points of the project. What worked or what could have been done to improve the project?).
- Reference Slide. (The week 3 Change Project Worksheet Systematic Review must be included in the references.) There should be a minimum of two (2) literature sources.
